These Terms of Service ("Terms") govern your use of the 360 Wellness website and your relationship with 360 Wellness ("we," "us," or "our") when you receive services at our med spa located at 459 Corporate Dr, Houma, LA 70360. By accessing our website or receiving services, you agree to these Terms.

Please read these Terms carefully before booking an appointment or using our website.

1. Services Overview

360 Wellness is a medical spa offering wellness, aesthetic, and healthy aging treatments in Houma, Louisiana. Our services include hormone replacement therapy, medical weight loss, body contouring, cryotherapy, laser treatments, cosmetic services, and restorative procedures.

All medical and aesthetic treatments are performed by licensed providers, including nurse practitioners, registered nurses, aestheticians, and other qualified professionals. 360 Wellness operates under the supervision of Brandon Rhodes, NP (FNP).

2. Appointment Policies

Scheduling

Appointments are available Tuesday through Thursday, 9:00 AM to 5:00 PM. We maintain a 15-minute buffer at both opening and closing to ensure each client receives full, unhurried attention from our team.

Credit Card Hold Requirement

A valid credit card is required to hold all appointments at 360 Wellness. Your card will not be charged at the time of booking. The hold guarantees your reserved time slot and ensures our providers can dedicate their full attention to your care.

Paid Consultations

Initial consultations at 360 Wellness are paid appointments. The consultation fee covers a thorough evaluation of your goals, medical history review, and a personalized treatment recommendation from our providers. Consultation fees may be applied toward the cost of subsequent treatments at the discretion of 360 Wellness.

3. Cancellation and No-Show Policy

We understand that plans change. 360 Wellness asks that you provide reasonable advance notice if you need to cancel or reschedule your appointment.

  • Cancellations with notice: Please cancel or reschedule at least 24 hours before your scheduled appointment time. Cancellations made with adequate notice will not incur a fee.
  • Late cancellations: Appointments cancelled less than 24 hours before the scheduled time may be subject to a cancellation fee charged to the credit card on file.
  • No-shows: Failure to appear for a scheduled appointment without prior notice may result in a no-show fee charged to the credit card on file. Repeated no-shows may result in the requirement of prepayment for future bookings.

360 Wellness reserves the right to modify cancellation fees and policies at any time. Current fees will be communicated at the time of booking.

4. Payment Terms

Payment for services is due at the time of treatment unless otherwise arranged in advance. 360 Wellness accepts major credit cards, debit cards, and other payment methods as posted at our location.

Pricing for services is subject to change without prior notice. Quoted prices are valid at the time of the quote and may not reflect current pricing if the appointment is booked significantly in advance.

5. Medical Disclaimer

360 Wellness provides medical spa services for wellness, aesthetic, and healthy aging purposes. Our treatments are not a substitute for emergency medical care, primary care, or treatment of acute medical conditions.

Important: If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. Do not contact 360 Wellness for urgent or emergency medical situations.

By receiving treatments at 360 Wellness, you acknowledge that:

  • Results from aesthetic and wellness treatments vary by individual. No specific outcome is guaranteed.
  • You are responsible for disclosing your complete medical history, current medications, allergies, and any conditions that may affect treatment safety.
  • Your provider will discuss potential risks, benefits, and alternatives before any procedure. Informed consent is required prior to treatment.
  • 360 Wellness practitioners will exercise professional judgment in recommending or declining treatments based on your health profile.

6. Informed Consent

Before receiving any treatment at 360 Wellness, you will be asked to review and sign an informed consent form. This form will describe the specific procedure, expected outcomes, potential risks and side effects, and aftercare instructions.

You have the right to ask questions and receive clear answers before consenting to any treatment. You may withdraw consent at any time prior to the start of a procedure.

7. Limitation of Liability

To the fullest extent permitted by Louisiana law, 360 Wellness, its owners, employees, and providers shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our website or services.

Our total liability for any claim related to services provided shall not exceed the amount you paid for the specific treatment giving rise to the claim. This limitation applies regardless of the legal theory under which the claim is brought.

Nothing in these Terms limits liability for gross negligence, willful misconduct, or any liability that cannot be excluded under applicable Louisiana law.

8. Website Use

Acceptable Use

You agree to use the 360 Wellness website for lawful purposes only. You may not use our website to transmit harmful code, attempt unauthorized access to our systems, or collect personal information of other users.

Intellectual Property

All content on the 360 Wellness website, including text, images, logos, graphics, and design elements, is the property of 360 Wellness or its licensors. You may not reproduce, distribute, or create derivative works from our website content without written permission.

These Terms are governed by and construed in accordance with the laws of the State of Louisiana, without regard to conflict of law principles. Any disputes arising under these Terms shall be resolved in the courts of Terrebonne Parish, Louisiana.

If any provision of these Terms is found to be unenforceable or invalid by a court of competent jurisdiction, that provision will be limited or eliminated to the minimum extent necessary. All remaining provisions will continue in full force and effect.

360 Wellness reserves the right to update or modify these Terms at any time. Changes take effect when posted on this page with a revised effective date. Your continued use of our website or services after changes are posted constitutes acceptance of the updated Terms.
